What Is Estradiol Hemihydrate — and How Is It Different?
Estradiol is available in several pharmaceutical forms — Estradiol Valerate, Estradiol Hemihydrate and micronised Estradiol are the most commonly used. Estradiol Hemihydrate is the form used in Estrafleg. It is a crystalline salt form of estradiol with excellent oral bioavailability — absorbed efficiently through the gastrointestinal tract and converted directly to 17β-estradiol (the biologically active form) in the body.
A large-scale retrospective study conducted at Nova IVF Fertility, Ahmedabad — involving 2,529 Indian women — directly compared Estradiol Hemihydrate with Estradiol Valerate in HRT-FET (hormone replacement therapy — frozen embryo transfer) cycles. The results showed that Estradiol Hemihydrate achieved a mean endometrial thickness of 9.57mm compared to 9.25mm with Estradiol Valerate, with no significant differences in clinical pregnancy rate, abortion rate or live birth rate. The clinical conclusion: Estradiol Hemihydrate is at least as effective as the established standard — and may offer marginal endometrial advantage.
Clinical Indications for Estrafleg
IVF — Endometrial Preparation for Embryo Transfer
The most critical use of Estrafleg in assisted reproduction. Before a frozen embryo transfer (FET), the endometrium must be adequately prepared and thickened to support implantation. Estrafleg oral tablets are prescribed from the start of the cycle, building the uterine lining to the optimal thickness of 8mm or above — the threshold at which implantation success rates are significantly higher. It may be used orally, or vaginally for enhanced local absorption in resistant cases.
Hormone Replacement Therapy (HRT) — Menopause Management
Menopause brings a sharp and sustained fall in estradiol. Hot flushes, night sweats, vaginal dryness, disturbed sleep, mood changes, joint pain and cognitive difficulties are all oestrogen-withdrawal symptoms. In women with secondary amenorrhoea (absent periods due to hormonal imbalance), Estrafleg is used as part of a sequential hormone replacement protocol — restoring the oestrogen phase of the menstrual cycle and facilitating withdrawal bleeding when combined with a progestogen.
Osteoporosis Prevention

When the Gel Is Preferred

Important Clinical Considerations for Prescribers
Always co-prescribe a progestogen in women with an intact uterus using systemic estradiol. Unopposed oestrogen stimulates the endometrium and increases the risk of endometrial hyperplasia. A progestogen (such as Dydrogesterone or Micronised Progesterone) should always be included in any HRT regime for a woman who has not had a hysterectomy.
Monitor estradiol levels and endometrial thickness regularly during IVF endometrial preparation cycles — dose adjustments may be needed based on individual response.
Do not stop abruptly in HRT patients — gradual tapering is recommended when discontinuing long-term oestrogen therapy to avoid withdrawal symptoms.
